Trivium’s long term support services offer individuals with developmental disabilities, intellectual disabilities, chronic mental illness, and brain injuries opportunities to reach their full potential at home, at work, and in their communities.
These services include:
Supported Community Living
Trivium’s Supported Community Living program provides individualized support for individuals who want to live independently while receiving the guidance they need to thrive. Services are offered in shared homes of up to four individuals or through flexible hourly support for those living in their own homes or apartments. Assistance may include medication monitoring, transportation, household tasks, and support in building meaningful community connections.
Supported Community Living services are available at all Trivium Iowa locations, and we are currently accepting new referrals.
Day Habilitation
Trivium’s Day Habilitation (LINKS) program provides a structured, person-centered environment where individuals with developmental or intellectual disabilities build independence and meaningful community connections. Through engaging daily activities, community outings, and skill-building opportunities, participants grow in confidence and strengthen independent living skills with the steady support of trained staff.
Day Habilitation services are available at all Trivium Iowa locations, and we are currently accepting new referrals.
Intensive Residential Service Home (IRSH)
Trivium’s Intensive Residential Service Home (IRSH) provides 24-hour, community-based support for adults with intellectual or developmental disabilities and behavioral health needs. In a safe home setting with no more than four individuals, each person receives personalized guidance, connection to services, and support to build stability and greater independence.
IRSH is designed for those who benefit from consistent supervision but do not require inpatient care. This service is currently offered at our Council Bluffs, IA location.
