Trivium’s Day Habilitation (LINKS) program offers a structured, person-centered environment where individuals with developmental or intellectual disabilities are supported in building independence, meaningful relationships, and strong community connections. Each day, participants choose from engaging activities designed to foster growth both on-site and throughout the community.
Through community outings, volunteer opportunities, and skill-building experiences, individuals strengthen independent living skills and gain confidence in their abilities. With the steady guidance of trained staff, each person is encouraged to pursue personal goals, discover new interests, and continue growing in a welcoming and inclusive setting.
Day Habilitation services are available at all Trivium Iowa locations, and we are currently accepting new referrals.
Admission Criteria
- Not harmful to others
- Funded by an outside source/private pay
- Not listed on the sexual offender registry
- Provide own living expenses if accessing daily SCL services
- Receiving/benefitting from services for which the individual applied
- You must be at least 16 years of age or older if you are not a student. If you are a student, you
must be 14 years of age or older and be deemed ready for the Day Habilitation program by the
School Interdisciplinary Team. - You must not be involved in illegal alcohol use (i.e., underage drinking), alcohol abuse or illegal drug use or must be actively involved and attending a substance use disorder program or support and be willing to transition to local support services, as necessary.
- If you have a history (recent history will be weighed heavily) of being a danger to self or others, acceptance will be contingent upon having an agreed upon written Behavior Intervention Plan (BIP) that Trivium staff can effectively implement, execute, and manage.
- If you have a felony on your record:
- Felony on your record cannot be greater than Class D felony
- Felony will not be considered if the involved crime was against a person and/or weapon used;
only non-violent felonies will be considered - Trivium maintains the right to refuse services to individuals with felony records if admission to
services could create an unsafe environment for staff providing services or clients participating
alongside them in services. Trivium also reserves the right to complete background checks, as
needed.
