Trivium offers two levels of Supported Community Living services designed to meet the daily living needs of adults with disabilities and mental illness. Each level of support is thoughtfully determined in partnership with the individual, guardians, case managers, and providers to ensure the right balance of independence and assistance — while also providing peace of mind for families and caregivers.

Residential Services

In our full-time Residential Services, individuals live as independently as possible in a home with two to three roommates, with staff support available up to 24 hours per day based on individual needs.

We believe home should feel like home. Individuals choose the house, duplex, or apartment that feels right to them. Each residence offers three or four bedrooms, with one person per bedroom to ensure privacy and comfort. Before moving in, potential roommates meet to determine if they are a good fit, helping foster respectful and supportive living environments.

Support levels are flexible and guided by each person’s service plan. Depending on the needs of those living in the home, staff may be available onsite up to 24 hours per day, with adjustments made as needs evolve. Individuals are encouraged and empowered to make choices based on their preferences, interests, and goals. To ensure access for those in need, Trivium opens new homes as referrals increase.

Hourly Support

For individuals who live with family or in their own homes, Trivium also provides flexible hourly services. Support may include medication monitoring, transportation to medical appointments, assistance with cooking and cleaning, and opportunities to build meaningful community connections. Whatever a person’s level of need, our team is here to provide steady, respectful support. (Hourly Support are not offered at our Council Bluffs location.)

Admission Criteria

  • Not harmful to others
  • Not listed on the sexual offender registry
  • Funded by an outside source/private pay
  • Provide own living expenses if accessing daily SCL services
  • Receiving/benefitting from services for which the individual applied
  • 18+ years old to access Daily SCL services; access to hourly SCL or respite as a child/adult
  • Additional criteria (per Trivium policy) will be considered at time of inquiry/referral
  • You must not be involved in illegal alcohol use (i.e., underage drinking), alcohol abuse or illegal drug use or must be actively involved and attending a substance use disorder program or support and be willing to transition to local support services, as necessary.
  • If you have a history (recent history will be weighed heavily) of being a danger to self or others, acceptance will be contingent upon having an agreed upon written Behavior Intervention Plan (BIP) that Trivium staff can effectively implement, execute, and manage.
  • If you have a felony on your record:
    • Felony on your record cannot be greater than Class D felony
    • Felony will not be considered if the involved crime was against a person and/or weapon used;
      only non-violent felonies will be considered
    • Trivium maintains the right to refuse services to individuals with felony records if admission to
      services could create an unsafe environment for staff providing services or clients participating
      alongside them in services. Trivium also reserves the right to complete background checks, as
      needed.
